We can put satellite offices of Medical onto the Starfleet Command campus. That makes sense, after all.

Roman was trained in the main starfleet command campus but that was before the war. Now things have changed so I guess I should have read up more about where Medical was located

I believe the IC reasoning behind it was after the destruction of Fleet command in San Fran during the war that SF didn't want a repeat of that. So a decentralized SF command happened. The main command branch in SF and other bits and pieces spread out across the globe.

Still would make sense to have some offices in San Francisco, though, especially with transporter rations limited.

I would imagine that just about any Starfleet facility would have an attached hospital or infirmiry. Training and overall administration would be handled in Sydney, however. Am I right?

I would tend to expect training and admin/command to be in Syndey, with more specialized groups being placed either in the core facility in Sydney or in their own offices located wherever seemed most logical (like the Intensive Trauma Team being attached to Starfleet Command to give more immediate access to patients from both directions).
